The reason why it sounds off is because the game is running on an incorrect speed which is 60hz.
50hz is the correct speed of the Master System port of Pac Mania since it was only released in Europe only.

i'm sorry but this is honestly the worst version of the ost
block town is completely off tune and it of course doesn't even have sandbox land like none of the other ports do
